Here's my demo code for what I was able to get to as well as what I wanted to get to. This is just a modification of the Community Server 2007.1 SDK that you can download here: http://get.communityserver.org/download/moredownloads.aspx

To run the demo you will need to merge the changes with the SDK using a merge tool like WinMerge. You can download WinMerge from here: http://sourceforge.net/project/showfiles.php?group_id=13216

I've also added a c# class library project you will need to add to the SDK's solution if you would like to edit it in visual studio. The class library contains the custom NameSubForm that I wasn't able to get to during the presentation.
